Avon Valley practice is well established having started before the NHS existed. The practice now operates across two surgeries along the Avon Valley. The main surgery is based in Upavon and the branch surgery in Durrington.

We are proud of the services we provide and hope we are able to fully support the health needs of you and your family. Treatment is available both on the NHS and privately for a wide range of general medical problems. Please refer to the Patient Guide to GP Services for further information.

Summary Care Record   

On the 16th April 2012, NHS Wiltshire will be sending information to all patients over the age of 15 years and 9 months, about the Summary Care Record (SCR).

The SCR will give healthcare staff access to key health information, which means that can provide safer care and treatment to patients in an emergency, for example, when providing care in out-of-hours services. A SCR will hold limited essential information such as details of any medicines the patient is taking and any allergies or adverse reactions to medicines they have had.
